2026-2027 Extracurricular Physical Information

GISD Families,
Thank you for taking the time to complete the athletics/fine arts participation paperwork online. If you do not have a parent/guardian account, please create one as you navigate through the https://georgetownisd.rankone.com website.

To get started, you will need your student’s name and ID number that was provided by Georgetown ISD.

Please note: If your child was enrolled in Georgetown ISD after mid April, it is possible that they may not be in our database. If that is the case, please read the information regarding new students at the bottom of this page.

Directions

All the forms are required to be submitted prior to participation in any practice, scrimmage, performance, or contest before, during, or after school, but they may be filled out and submitted at different times. If you are waiting to have a physical done, you may still complete and submit the other forms.

Physicals must be dated after May 1, 2026 to be valid for the 2026-2027 school year. Please print the form to take to your physician from this link https://sites.google.com/georgetownisd.org/ghsathletictraining/26-27-physical-information?authuser=0 which can also be accessed by selecting the download and print tab located on the RankOne Georgetown ISD parent page.

* Once the physical is complete, please upload the physical to the RankOne Georgetown ISD portal under the “Completed Physical Upload Form” section.

The medical history and physical submissions must be reviewed by a district athletic trainer. Please allow us time to review/deny those forms. The training room is closed for the Summer, but we check on the forms periodically to approve/deny them.

*IF A FORM IS DENIED, YOU WILL GET AN EMAIL/ PUSH ALERT NOTIFYING YOU OF THE REASON FOR THE DENIAL.

If the physical form is unable to be submitted as an attachment on the RankOne website, it can be emailed to the email address designated below for the 2026-2027 school year. If you have already submitted the form via the preferred method on the RankOne website, please do not submit via email. Submissions to multiple locations and email submissions may take longer to review and process the physical.

• EVHS, TMS, WMS students please submit the completed physical to [email protected]
• GHS, BMS, FMS students please submit the completed physical to [email protected]

**Students New to GISD in the 26-27 school year*
The school district will rollover and automatically add new students into the system in mid July. There is a chance that new students who have enrolled in the district in mid April may not yet be showing up in our database. If you have the completed physical, dated after May 1, 2026, please submit it via email to the email address listed above. PLEASE INDICATE THAT THE PHYSICAL IS FOR A NEW STUDENT in your email. If you need a copy of the physical for a camp, please provide a paper copy to the coaches for these camps. Return to the rankone website to fill out the required paperwork in mid July.. We will upload the physical form for you if you have sent it to us.
